Tidelands Georgetown Memorial Hospital recognized for age-friendly care provided to older adults
Tidelands Georgetown Memorial Hospital has earned recognition for taking extra steps to help older patients receive high-quality care catered to their unique needs.
The Georgetown hospital earned the highest designation as an Age-Friendly Health System – Committed to Care Excellence for meeting strict standards in the delivery of health care to older adults. Through data and a rigorous review, Tidelands Georgetown confirmed its commitment to providing age-appropriate care using the evidence-based 4Ms framework model.
The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States, developed the Age-Friendly Health System initiative to help meet the needs of the growing population of older adults nationwide. Tidelands Waccamaw Community Hospital in Murrells Inlet was named an Age-Friendly Health System participant in 2020.
“Older patients find personalized, evidence-based care provided at Tidelands Health that is tailored to their unique physical and emotional needs. These customized care plans are centered on what matters most to our older patients,” said Ashley Capps, vice president of operations and chief nursing officer at Tidelands Health. “The age-friendly designation is a testament to the level of personalized, evidence-based care our team provides every day.”
To meet the initiative’s criteria, Tidelands Georgetown demonstrated, through data, its commitment to providing care for older adults using the 4Ms framework model by focusing on what matters to the patient; using age-friendly medication, when needed; preventing and managing conditions including dementia and depression; and maintaining mobility by helping older adults move safely every day.
