News for Immediate Release:
Tidelands Health has received a grant to help uninsured and underinsured residents get access to the health care they need.
The Leon Levine Foundation awarded a $50,000 grant to Tidelands Community Care Network to increase enrollment in its care-coordination program. Tidelands Health, in partnership with AccessHealth SC and The Duke Endowment, created Tidelands Community Care Network to break down barriers to medical care for uninsured and underinsured adults.
“This generous grant from The Leon Levine Foundation will directly help members of our community gain access to the medical care they need to live better lives through better health,” said Kelly Kaminski, senior director of community health resources at Tidelands Health. “Our team will be able to use this grant to enroll even more residents in Georgetown and Horry counties who can benefit from the care coordination and improved access to health care that our network of community partners can provide.”
Since its inception, Tidelands Community Care Network has served more than 5,600 individuals by connecting them with primary care, specialty care, affordable medication, behavioral health care, dental care and care of chronic conditions such as diabetes.
With nearly 50 partner agencies and organizations, the network helps remove barriers such as a lack of transportation to needed care. One of the network’s greatest community benefits is its role as a facilitator helping to foster partnerships and increase cross-sector collaboration. The result is stronger links within the community that lead to integrated care and better health outcomes.
In awarding the grant, The Leon Levine Foundation applauded the Tidelands Community Care Network’s case management structure and the collaborative nature of its safety-net community.
“We are impressed by Tidelands Community Care Network and its highly effective, comprehensive community of support which provides positive outcomes for the region,” explained Tom Lawrence, president of The Leon Levine Foundation. “We are proud to help fund an organization so closely aligned with our mission of providing access to high-quality health care for those who may otherwise have no alternative for essential services.”
Based in Charlotte, North Carolina, The Leon Levine Foundation was created in 1980 by the founder and chairman emeritus of Family Dollar Stores Inc. and supports programs and organizations that improve the human condition. Health care and human services are two key areas the foundation supports through investments in non-profits across the Carolinas with strong leadership and a track record of success.
“We are grateful to The Leon Levine Foundation for recognizing the important work of Tidelands Community Care Network and supporting our efforts to serve our community,” Kaminski said.
